Member Information

FLOTILLA 12-42 MEMBERS:

The following are two important Auxiliary electronic forms that each flotilla member should complete for timely reporting of information to AUXDATA.

Form 7029H - MEMBER ACTIVITY LOG - New and mandatory as of 01Jan2010

This form is used to report all activities not reported on other AUXDATA reports. Include study time for classes, travel time to meetings, preparation of FSO reports, etc. Click on the link above and complete the form, following the included instructions.  Note: The Email version of the new form 7029H will be available soon.

  • Enter your email address on the bottom left of the page.
  • Enter the FSO-IS, Rick Shiffman's, email on the bottom right for "RECIPIENT" (rrs0@earthlink.net).  You may add your own email address and that of others as well.
  • Choose the form view you want from the drop-down menu.
  • Click "SUBMIT" to automatically email a copy of the form to the FSO-IS.
  • Click "PRINT" to reproduce a copy of the form for your records, if you like.

Form 7030 - MISSION ACTIVITY REPORT - Updated March 2010

This email form is used to report all missions, such as public education classes and on-the-water patrols under orders. When more than one member participates in a mission, only the Lead Member should complete the form. Click on the link above and complete the form, entering the FSO-IS email address and your and/or the lead member's email address, as appropriate. The process is the same as that detailed for the 7029H form above.
